Application Checklist

• the completed application and $50.00 application fee have been sent to St. Patrick's School



• a copy of the baptismal certificate has been included in the application package



• a signed transcript release form has been sent to the child's current school



• the child's current teacher has been given the recommendation form



• a visiting day at St. Patrick's has been set up for the applicant
